Perhaps you are only familiar with Google captchas? (which are a PITA and privacy invasion).
I use an onsite captcha on my website. It adds the question How many letters are in the word "bleah" ? to all user-submittable forms and checks for the correct answer. Question and answer are easily changed. It is very simple but eliminates spam submissions by virtually 100.0%. It is completely private and accessible to blind users.
Yes, it can be fooled by automation, but most sites will not be attacked by that often. Additionally the simple captcha software adds hidden form fields which should not be filled, which stops most bots.
Captchas like these are readily available plugins, however ignorant and lazy website operators fail to use them.
