Untitled unsolicited file stuck in download queue?

This afternoon as I picked up the phone which had been lying idly, it showed a notification from system downloader which said something like:

<Untitled>
Download Error

In Files -> Downloads, there was an unnamed file of type "file", the info of which only said "queued for download". I deleted that file after several unsuccessful attempts of extracting any more info.

Neither RethinkDNS nor the system logs seemed to have entries for that time (at least meaningful ones, except maybe for ntfy app in RethinkDNS?).

Auditing the device shows it's untampered with (yay!).

Unfortunately due to a reboot in between the logs now seem lost.

What could cause this, and how would I properly investigate if it occurs next time?

Emmy wonder if its one of those annoying, obnoxious ghost files? @Novaliss 's trick works amazingly. You could try that

Developer-Dude possibly, although I could delete it without that trick. And those ghost files were traced to Rethink DNS, an app I'm also using, which doesn't use the system download manager in my case.

So more than anything else, I'm wondering what made it occur?

If something starts downloading stuff without my doing and knowledge, there's an obvious security implication for the device (knowing the OS should catch almost anything anyway).

Emmy try erase RethinkDNS logs, both network logs as well as DNS logs can be deleted with no loss of rules. I got a couple of issues solved by doing this.
